Eagles Eye Mobile Powered by Vision to Learn

**We are currently fully booked for Summer 2024. If you are interested in joining the waitlist for an appointment, please fill out this form and we will reach out if appointments become available. 

Exams are only available for children ages 5-18 (Grade K-12). The eye exam will be 20-30 minutes. Please arrive 10 min before your appointment to fill out paperwork. Please expect that the wait time may be up to 30 minutes. 

Children 17 and under must be accompanied by an adult.

**If applicable, vision services provided will be billed to your child’s Medicaid benefits. There will be no out-of-pocket cost.**

Este examen de la vista es solo para los niños de 5 -18 años (kinder - grado 12).
